New Online Medicaid Complaint Form


The Agency for Health Care Administration (Agency) implemented a new online Medicaid Complaint Form on June 1, 2018.  

The new form was developed to:

  • Give submitters a complaint tracking number for each complaint submission,
  • Solicit better information up-front, for faster processing,
  • Reduce the time needed for a complaint to be sent to a Plan (if applicable), and
  • Reduce the total time needed for complaints to be worked, from start to finish.

In addition, the Agency has implemented a new Medicaid Complaints webpage, which was developed to give providers and recipients better information about the Agency’s complaint process, to answer commonly asked questions, and to offer some additional complaint resources.

Providers can also choose to submit complaints by phone, by calling the Medicaid Helpline at 1-877-254-1055 (TDD 1-866-467-4970).  Staff are available to assist you Monday through Friday, 8am-5pm EST.
